Contracts for Professional Services (CPS)The Contract For Professional Services (CPS) form documents contractual relationships with an individual, partnership or corporation to provide a professional service. Professional Services are defined as customized services consisting of specialized or creative expertise based on personal skills or ideas of an individual(s) that are provided for a fee, which may be determined individually with each customers for each service contract. Below is a list of forms used in the CPS process.

CPS Attachment: Travel Reimbursement for Contractors
This is an OPTIONAL tool intended as a guideline to help departments incorporate language into the contract that will establish limits on travel expenses reimbursed to contractors, especially to out of town contractors performing lengthy projects on-site at the University. Departments are encouraged to add this attachment to their RFP and/or CPS. The language may be incorporated into a final contract, whether or not the contractor is selected through the RFP process. Language on travel expense reimbursements are negotiable with the contractor, so the language in this attachment may be changed to be more or less stringent.

Administering Contracts
Contract administration is the management of a contract after the
contract is executed, (in most cases signed by both parties), and
the vendor has been sent a purchase order or a contract for professional
services CPS).

Amending Contracts For Professional Services
Occasionally, department may find it necessary to make a change or extend an existing Contract for Professional Services (CPS).
Any changes in a CPS that require documentation should be done on a Contract for Professional Services Amendment. Departments should be aware that completing a new CPS rather than amending a standing contract is incorrect, and that this practice was a significant finding in the 1998 Internal Audit. Minnesota New Hire Form
Effective 7/01/96, Minnesota State Law required the reporting of all
new/rehired employees AND independent contractors to the State of
Minnesota within 15 days of hire. The "New Hire Reporting
Form", GS92355 (BA 1457) must be completed for all, but the following
reasons: Reimbursements, Per Diem, Housing Allowances, Services
out of the Country.

Imaging
Departments use the Imaging system to archive their completed CPS documents and any related attachments or support documentation. There are two coversheets used to fax the CPS documents into the imaging system. One is for the original document and its attachments. The other is to fax in CPS amendments. Training Services has a job aid available for information about imaging the CPS and amendment documents.

Professional Services Information Sheet
Section 1 of the Professional Services Information Sheet (PSIS) must be filled out and attached to every CPS Requisition - Other sections conditionally required.
